St Michael's Hospice Dementia Service provides specialist support from diagnosis to end of life. We deliver expert clinical care, carer support, therapeutic activities and access to wider hospice services, enabling people with dementia to live well, avoid crisis and die with dignity.

Over 3,100 people are thought to be living with dementia in Herefordshire, with numbers rising. Dementia is life-limiting, yet access to palliative care is inconsistent. Carers face isolation, emotional strain, and complex systems. Without early support, crises lead to hospital admissions and poor end-of-life experiences. Current services cannot provide continuous, holistic care across the full dementia journey. This is particularly poignant at Christmas; we want to ensure no-one is forgotten.
Our Dementia Nurse Service provides continuous, specialist support from diagnosis to end of life. We will deliver home visits, symptom management, carer training, peer support, and coordination of care. We will enable access to wider support with Hospice at Home allowing for patients to die with dignity in their home, surrounded by family. Our holistic solution will reduce crises and ensure people with dementia and their carers receive compassionate, coordinated support throughout their journey.
