Hospice at Home aims to offer additional support, alongside the community nursing, to allow people to stay at home and receive personalised high-quality end-of-life care, as well as supporting those important to them.

Over 70% of people wish to die at home, yet only 30% in Dorset do. Without 24/7 specialist care, patients face unwanted hospitalisation, and families are left unsupported. The impact of not having this service removes individual choice, leaving people without dignified, personalised end-of-life care in their preferred setting. It denies them the choice to spend their final moments in the place they feel safest, and surrounded by the people, pets and places they love most.

Hospice at Home will provide end-of-life care, 24/7, to patients at home. This fills a critical gap in Poole, supporting both patients and families, and ensuring people can die with dignity in a place of their choosing. It will enable more people to die at home whilst receiving the same high level of expert care that they would receive in our hospice. This aligns with our mission to ensure that everyone gets the care and support they need, either at home or in our dedicated hospice facility.
