Life with a life-limiting illness during winter can be fraught, uncertain and hard practically and emotionally. Especially if you want to stay at home. Our region has the highest number of people on the End-of-Life Register in England, meaning doctors believe they are in the last year of life. Through our innovation at Garden House we are reaching more people, with more illnesses. Hundreds of patients and their loved ones will need us this winter when they hit a crisis or need end-of-life care.

Our community team comprises nurses, health care assistants and admin support. They spend winter visiting people at home. They make plans to best support patients, help families cope, escalate issues, advocate, give care. They relieve pressure on hospitals. As a team they support each other, working together for every patient’s unique needs. They may pop Christmas dinner in the oven. They may be the only person a patient sees. United they keep families together, home, safe, in peace and comfort.
