In rural areas, individuals with life-limiting conditions and their carers face isolation and struggle to access care due to distance, poor transport links, and NHS pressures. With most hospitals in urban areas, regular support is harder to reach. It is estimated that 55% of those in the last years of life reported not knowing where to find information on how to plan or support a loved one. Informal carers provide as much as 75-90% of homebased care for people who are near the end of life.
We bridge this gap to ensure no one feels alone. Our team provide expert medical support, manage pain and symptoms, and offer holistic person-centred care. We join the dots between families, patients, and doctors, and support care plans and provide emotional support. We educate families about end-of-life care, empowering them to be involved.
