Sage House aims to support all in our community who are affected by dementia. Through the services at Sage House, and through our Community Outreach and advice bus, we are bringing dementia support to the local community, helping more people access personalised support and reducing isolation.

Situation
Over 17,500 people have dementia in West Sussex, predicted to increase by 36% by 2030. Navigating daily life with dementia, as well as the health and social care system, can be challenging as it is fragmented and complex. The services at Sage House improve the quality of life and wellbeing for thousands of people, however for some the lack of local public transport or the loss of a driving license can cause isolation and make services difficult to access given the rurality of the area we cover.
Solution
Providing connected, personalised and impactful services at Sage House and in local communities will help improve quality of life and mental health for people with dementia and their carers. We will provide regular sessions at community venues across West Sussex, including engaging activities, group discussions to enable peer support, and 1:1 advice and guidance to help navigate life with dementia. Our advice bus service will enable people to access dementia support in their local community.
