Our ‘Hospice at Home’ service supports patients and their families following a palliative diagnosis, ensuring no one faces this difficult time alone. Our team provides holistic, joined up care, helping patients avoid distressing emergency hospital visits and allowing people to die ‘well’ at home.

Situation
After a palliative diagnosis, many people receive little or no support until they’re near the end of their life which is very isolating. Some face distressing, costly, emergency hospital admissions (one ambulatory admission and one overnight stay through A&E costs the NHS on average £3,459) in their last weeks, meaning they spend their final days in hospital instead of at home with loved ones. In other cases, patients may receive poorly coordinated home based care which can be very challenging.
Solution
Following a successful pilot we’ll deliver our ‘Hospice at Home’ service in Poole and Purbeck, collaborating with GPs and other healthcare professionals to anticipate issues before they occur and enhance the provision of end-of-life care. We will visit patients in their own homes, discuss end of life plans with them, offer respite sits for carers, co-ordinate their care, and give access to our other services such as day hospice, counselling, occupational and complementary therapy. .
