    Situation

The numbers of people sleeping rough in London is the highest we've seen in over 2 decades. The link between mental health and homelessness is well documented but with rising caseloads statutory services struggle to reach let alone support this group of people who have complex needs. It is crucial to reach these individuals with appropriate support. The Passage's Outreach and Engagement team works to build trust and engagement and provide support for individuals before they deteriorate further.

    Solution

The service has been built on the challenges identified for this group. The team work in trauma informed way to tailor support to the individual. The team works with experienced multi-disciplinary team of mental health professionals and its two complex needs case workers to help put in place support to facilitate better outcomes for homeless people. As part of the service we work towards recovery we also support them into safe and sustainable accommodation at a place they are comfortable with.
