    Situation

    The wish of patients to die at home with loved ones in familiar surroundings, rather than in hospital, is a choice that many want, but it is often unavailable. People are scared of dying alone and family members can be unsure how best to support their loved ones through a terminal illness. Patients often find it difficult to express their final wishes and talk openly about death with family. We will expand our coverage southwards to include Morpeth, thus reaching a greater population.

    Solution

    By working closely with the district nurses, allied health professionals & GPs across North Northumberland, our skilled palliative care team will continue to deliver person-centred care to ensure each individual is treated with dignity and respect, with an emphasis on living well to the end. We will expand the emotional and practical support for families and carers via our bereavement support service. Our nurses and carers provide care enabling the final days to be peaceful and comfortable.
