We are using our skills to launch a new frailty service, so more people can access us, benefit from our care and live a better life. It will reduce hospital admissions at the peak of winter and could save the NHS £2m. This is a new chapter for our hospice. We are ready but we need your help.

Situation
More people live with multiple health issues such as high blood pressure, heart failure or dementia, which reduces their independence and makes them vulnerable to illnesses like colds resulting in hospital admissions – possibly many times a year. This is frailty. It’s bad for the person, bad for the NHS and a global issue. Also, there is inequality in who accesses hospices. Locally 5000+ people are eligible for our care but not receiving it. Many of those have frailty. It’s time we changed this.
Solution
At Garden House Hospice Care are experts in end-of-life care and symptom control. People are arriving at their GP or A&E experiencing breathlessness, pain, or having fallen. We have the experience and knowledge to support people with these symptoms. Our plan is to extend our support to people with frailty so they can live well at home. We will open additional beds to support the NHS over winter when admissions will rise, expand our community services and provide new treatments at the Hospice.
