We aim to support Nottinghamshire's adults with learning disabilities & their families to plan & prepare for brighter futures. Our expert staff will support vulnerable adults & their unpaid carers to gain vital skills & knowledge proven to reduce risk of loneliness, social isolation, & poor health.

    Nottinghamshire’s population of adults with learning disabilities is set to rise to c.16,600 by 2035; they & their families urgently need timely, expert support to reduce risk of very poor health, social & economic outcomes now & in the future; issues include: - more people growing older with learning disabilities still living with parents in 70s & 80s, often unknown to services, & with no plan for the future. - carers are exhausted - lack of support for younger adults leaving school/college.

    3 expert workers will unite with adults with learning disabilities & family carers to create bespoke, holistic a programme of activities that promotes planning for more independent futures, including - 240 Independent Life-skills Sessions for adults with learning disabilities preparing to transition at key life-stage, eg into adult services, moving home - 100 one-to-one & group planning support sessions for unpaid family carers - Outreach work to find those in urgent need & unknown to services.
