We are seeking support towards our Hospice at Home service. This service provides crucial end of life care nursing for individuals wishing to lead their final days in the comfort of their own home surrounded by family and all that is familiar. Giving people choice and dignity at the end of life.

Situation
Given the choice, most people would prefer to remain in their own home, surrounded by all that is comforting and familiar at the end of their lives, unfortunately this does not always happen due to lack of specialist support at home. Many patients need to be admitted to hospital or hospice beds without meeting their preferred place of death and with added psychological distress for their families. There is no access to Marie Curie or Macmillan nursing in this area.
Solution
Our Hospice at Home service is available to patients and their families during the last three months of their life. Crucially, it provides families with support and flexibility in times of crisis or emergency and enables the patient to stay at home in their usual, comfortable environment. When the patient is nearing the end of their life care is arranged via the hospice with the patients' family and other care agencies. Crucially this service gives patients choice and dignity in dying.
