    Situation

Around 90% of diabetes care is self-administered. This requires ongoing support in the provision of authoritative information and education throughout the long-term of the condition. Diabetes is a complex chronic condition with many associated complications. Good self-management skills are crucial to ensure that people with all types of diabetes live a long and healthy life whilst dealing with the day to day challenges that present themselves.

    Solution

By providing opportunities to spend time with specialist diabetes and related Healthcare Professionals, we are giving people with all forms of diabetes the chance to learn new self-management skills; refresh knowledge on an ongoing basis and develop relationships with people experiencing the same health issues. It is a fact that peer support plays a major part in helping people to take a pro-active approach to their self-care. The majority (at least 2/3) of the attendees are aged over 55.
