Many people with a palliative diagnosis receive little or no support until the final stages, leading to isolation and distress. Some face avoidable, costly emergency hospital admissions and spend their final days in hospital rather than at home with loved ones. Others receive disjointed care from multiple services without coordination, making it difficult to manage their condition and reducing quality of life.

Through our Hospice at Home service, we support people from the point of a palliative diagnosis, not just end of life. Our experienced nurses coordinate care with GPs and other providers, reducing isolation, emergency hospital admissions, and fragmented care. We offer holistic, person-centred support tailored to what matters most to each patient and their family.
